What do you hope cert holders get out of this presentation?

Healthcare roles will continue to evolve and expand as the industry shifts to a patient-centered model, like team-based care. As quality remains a primary focus, you must also empower your people to take an active role in the transformation. The industry needs solutions that will unify care teams and demonstrate competencies that are at the core of your practice. This session will provide an actionable framework to promote employee confidence and the skills needed to address the lack of resources and practice management tools dedicated to team-based care. All of which are key as you strive to increase business efficiency and empower the entire team to put patients first.

What is team-based care and what types of organizations are most likely to institute this model?

Team-based care is a care model where teams of healthcare professionals work together to ensure their patient’s needs are being met and the right care is being delivered, in the right place, at the right time, by the right person or healthcare professional. Most commonly this practice is occurring within organizations that are Patient Center Medical Home’s or Accountable Care Organizations but there are also many primary care and specialty offices are starting to adopt this effective care model.

How are organizations benefiting from implementing the team-based care model?

Providers are challenged with managing patient populations under legislation that demands improved quality care and they lack the resources they need to transform their practices. Team-based care (TBC) models provide a viable solution for providers to meet these increasing demands, and are expected to see continued growth. Organizations practicing a team-based care model may also see cost benefits from more efficient work flows, higher patient and provider satisfaction and enhanced patient outcomes.

Why is it important for allied health professionals to become familiar with this model?

For these models to be successful, practitioners like the nurse and medical assistants need to expand their knowledge and refine their skill set – setting them up for increased opportunities on these care teams. The goal is to keep patients as healthy as possible and simultaneously reduce cost by minimizing the need for expensive intervention. Allied health professionals can play a vital role in population health management by focusing on individuals at high risk. They may also have the opportunity to advance into other roles such as a health coach, patient advocate or patient navigator.
